Additional Information Submitted by Applicant
1. Date Submitted: 05/18/2023Hello, I have attached our application and have my answers in RED. I am having a difficult time finding a map that will meet the county's ask showing parking lots and streets labeled. Since we will only be using the parking lot in the south bay, do I need a map? If so, can I be directed to one that will meet the request and I can use that to indicate where we intend to host the event and park? We are very flexible with days, our main goal is just to be able to use the stage for our 10 year anniversary party and then allow our participants to hang out at the swim beach. Please let me know if theres anything we need to amend to be granted the permit. Thanks and I look forward to hearing from you. Matthew Lopez 516-287-3622

Comments: Please provide: 1. A certificate of insurance with event insurance/general liability limits of no less than $1,000,000 per occurrence / $2,000,000 general aggregate. The certificate of insurance must name Larimer County as the certificate holder (Larimer County 200 W Oak St Suite 4000, Fort Collins, CO 80525) and list Larimer County as additional insured. Please note that Larimer County does not accept additional insured language that limits additional insured status to that which is “required by contract”; however, "by agreement" is an acceptable addition. 2. A copy of the participant waiver. For questions about insurance requirements, please see the following informational page: https://www.larimer.org/special-event-insurance-information Thank youSheriff

Comments: I assume the "stage" is the pavilion next to the swim beach at South Bay. Defer to DNR Park Rangers as this affects weekend park activity and most likely doesn't require LCSO assistance/oversight.
