Description:
This is a Tuesday evening Mini-Tournament where we fish a catch and release Bass Tournament, launching around 4:45 p.m. and off the water by 9:00 p.m. The description of this event is identical to our other tournament permit (#6083) as we inadvertently omitted this date. A new insurance COI Additional insured will be requested. All State, County, and Federal rules and regulations will be followed. Life vests are worn by all contestants while big motor is engaged. No needs for additional staff or port-a-potties. Emergencies will be addressed and we have radio and cell phone contact with LC Rangers.

1. Date Submitted: 02/09/2023All tournaments are launched out of South Bay at Horsetooth Reservoir and parking will be contained in the South Bay parking lots. All of these tournaments are for competitive bass fishing. All competitors will have the Tournament Director's cell phone number in case of emergency, and the Tournament Director will have access to Ranger On Duty for that day. No additional restrooms or equipment will be needed. All activities will be at South Bay and on Horsetooth Reservoir.
All competitors must sign the attached waiver. COI to follow.

Comments: The Health Department does not have significant concerns with this event. We understand the event description is the same as the other event dates listed in special event #6083.-No food is being provided, therefore a 2023 Colorado Retail Food License or 2023 Larimer County Temporary Food License is not required.
-Water for drinking shall be from a public, treated water supply (which can be transported and dispensed in clean and sanitized food grade containers, or provided using commercial bottled water or from potable water taps onsite if conveniently located.
-If permanent restrooms do not contain running water, a temporary hand sink should be provided with soap, paper towels and a waste bin.
-Please ensure an adequate number of trash receptacles are accessible throughout the event set up area, including in food service areas.
-Please ensure all volunteers/participants are aware of the emergency communication plans.

Comments: A certificate of insurance with event insurance/general liability limits of no less than $1,000,000 per occurrence / $2,000,000 general aggregate. The certificate of insurance must name Larimer County as the certificate holder (Larimer County 200 W Oak St Suite 4000, Fort Collins, CO 80525) and list Larimer County as additional insured. Please note that Larimer County does not accept additional insured language that limits additional insured status to that which is “required by contract”; however, "by agreement" is an acceptable addition.Thank you.
